About Us
Building Healthier Communities, Together
Our Mission
We exist to enhance the ability of MyCHN to deliver accessible healthcare services to those who need them most. By addressing the healthcare gaps faced by uninsured and underinsured populations, we strive to improve health outcomes and advocate for equitable care for all.
Our Vision
We envision a future where every individual—regardless of insurance status—has access to comprehensive, high-quality healthcare services. By working toward health equity, we aim to ensure that all communities have the resources they need to thrive.
Improve Health Outcomes
Support essential healthcare services that enhance quality of life for underserved communities.
Foster Community Partnerships
Build strong relationships with local organizations and stakeholders to amplify our collective impact.
Advocate for Equitable Policies
Champion local, state, and federal initiatives that expand access to healthcare and promote health equity.
Why It Matters
The communities we serve are often the most vulnerable, facing significant barriers to accessing the care they need. By investing in innovative solutions, community programs, and advocacy efforts, the My Community Health Foundation helps bridge these gaps and create a healthier, more equitable future for all.
